I don't understand her problem

I have a friend who had have been friends with for years, in the early years of knowing each other we were the best of friends. She is a doctor and has always come over ever so slightly patronising and I accepted this thinking it was just her being a Dr and it was her bedside manner. Neither myself or anyone in our group of friends (there is 7 of us all together) have never said anything to her, as we always thought she'd be horrified if we confronted her about her behaviour. However she had got worse as the years go on. More recently our friendship between the two of us feels like it
