Mood swings in relationships

Hi, I'm a 30 year old male and I feel it's time for me to look for a solution to a problem that I've had for over 10 years now: Very inconstant relationships that I almost always end after a relatively short time (a few weeks). Most often I go through frequent mood swings in my feelings towards my girlfriend: heavy attaction is superceded by a great need for distance and even disgust (and the guilt which accompanies these feelings). I've wondered if this is some kind of personality disorder. If anyone has had any experiences with this and how to treat it, I would be very interested.
